Description Position summary: Under general supervision, learns to plan, organize or manage the activities of social service or community programs. Oversees the program or organizations policies regarding participant involvement, requirements and benefits.
Job Responsibilities & Performance Standards: - Supervises and plans work of assigned staff
- Effectively serves on and participates in program development team. Participates in strategy or review sessions, committees, and task forces.
- Thoroughly researches community/unit needs and/or resources for possible new program development. Effectively represents the CSB in planned community meetings.
- Develops and coordinates program or plays a strong role in program development.
- Presents program to unit, agency, outside agencies, customers and /or families.
- Develops necessary policies, procedures, criteria, and/or protocol(s).
- Composes well-researched, well-written, persuasive grant proposals.
- Helps assure program implementation, and appropriate linkage with other programs, units, and resources.
- Acts as client advocate to coordinate required services or to resolve crisis
- Performs intakes for customers who are at risk for homicidal, suicidal, or life-threatening behaviors. Evaluates and manages customers according to accepted standards of clinical practice.
- Determines necessity for involuntary commitment of customers in accord with accepted standards of clinical practice.
- Executes required forms/documentation for involuntary commitment according to accepted legal and clinical guidelines.
- Provides behavioral or therapeutic interventions for more intractable or difficult to manage customers.
- Counsels clients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life
- Evaluates suitability of customer for therapeutic intervention in accordance with accepted standards of clinical practice.
- Establishes realistic goals and timetable (Individual Service Plan) for therapy with customer, families and other treatment staff.
- Utilizes therapeutic techniques in accordance with accepted standards of clinical practice.
- Thoroughly and accurately documents the progress of therapy in relation to the customer's treatment plan.
- Analyzes results of treatment efforts for indications of progress and alters the therapeutic approach and treatment plan accordingly.
- Identify environmental impediments to client of patient progress through interviews and review of patient records
- Routinely conducts staffing meetings to discuss problems and resolutions, to assist staff in developing appropriate care plans and/or behavioral plans, and to explain departmental procedural/policy changes.
- Appropriately interprets, implements, and enforces compliance with policy/procedure and safety regulations.
- Reviews staff's treatment assessments, recommendations, and interaction with patients for appropriateness and provides effective, timely feedback.
- Reviews treatment documentation for accuracy and conformity in accordance with established agency guidelines.
- Accurately evaluates capabilities of supervised staff and assigns caseload to ensure that acceptable quality standards are maintained.
- Models effective counseling and crisis management techniques for supervised staff.
- Ensures that licensed staff maintain credentials to practice in their functional areas in accordance with accreditation standards, state, and federal laws.
- May administer and coordinate services and activities of a specialized program
- Thoroughly identifies and documents objectives and priorities based upon identified client needs, departmental policies, accreditation requirements, budgetary considerations and agency mission.
- Develops plans to meet identified objectives and priorities in accordance with established procedures.
- Effectively communicates objectives and priorities to superiors and subordinates.
- Promptly reviews and updates goals and plans to reflect changes in policy, identified needs, accreditation standards and communicates changes to staff in a timely manner.
- Effectively operationalizes directives and agreements from governing boards into functional program policies and procedures.
- Monitors, evaluates, and records client progress according to measurable goals described in treatment and care plan
- Meets with staff to discuss short/long range fiscal needs and plans for continuum of services within a specified time frame.
- Thoroughly evaluates and prioritizes budget requests based on client need and available resources.
- Accurately prepares and submits program budget(s) in accordance with established time frames and departmental operational guidelines.
- Effectively monitors budget for appropriateness of expenditure and fiscal accountability.
- Promptly reviews and approves requisitions from staff in accordance with established departmental procedures and guidelines.
- Thoroughly researches a variety of sources for optional funding, makes grant proposals and in-house requests for funds.
- Organizes support groups or counsel family members to assist them in understanding, dealing with, and supporting the client
- Defines goals and/or required results at beginning of performance period and gains acceptance of ideas creating a shared vision.
- Communicates regularly with staff on progress toward defined goals and/or required results, providing specific feedback and initiating corrective action when defined goals and/or required results are not met.
- Confers regularly with staff to review employee relations climate, specific problem areas and actions necessary for improvement.
- Evaluates employees as scheduled intervals, obtains and considers all relevant information in evaluations and supports staff by giving praise and constructive criticism.
- Recognizes contributions and celebrates accomplishments.
- Motivates staff to improve quantity and quality of work performed and provides training and development opportunities as appropriate.
- Performs clinical assessments of the behavioral health needs of consumers presenting for services
- Interviews applicants or employees to fill vacancies or promotional positions according to applicable laws, rules, and policies.
- Selects or promotes the appropriate number of individuals who possess the skills needed to perform the required work.
- Identifies training needs and ensures that necessary job-related instruction is provided to staff.
- Discusses potential grievance-related concerns with employees to identify options or resolve issues prior to the formal filing of a grievance.
- Advises employees of established grievance procedures.
- Recommends or initiates disciplinary actions according to applicable rules and polices.
- Provides case management services related to identified treatment needs
- Interviews patients and/or families to obtain necessary information regarding medical, social and/or emotional background.
- Consults with other health care professionals within or outside the organization to obtain necessary background information on patient.
- Completes a clear and concise Admission Note in the patient's chart within the time frame specified by hospital policy.
- Reviews patient's chart to obtain background information necessary to complete an assessment.
- Compiles information and completes a written assessment or update which is placed in patient's chart within the time frame specified by hospital policy.
- Ensures that psychosocial stressors and level of functioning are identified and documented in patient's assessment and/or Admission Note.
- Maintains confidentiality of patient information as specified by hospital policy.
- Provides training to direct care staff in implementing behavior modification techniques and treatment plans
- Attends treatment team meetings when scheduled.
- Participates in the development of a treatment plan and in the treatment plan review as required by hospital policy
- Involves the patient, family/significant others in goals and objectives that are identified in the treatment plan.
- Writes concise, accurate, clear progress notes in the patient's chart as required by hospital policy.
- Makes treatment members aware of the psychosocial stressors and level of global functioning identified in the patient's Assessment and/or Admission Note.
- Refers client or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as financial assistance, legal aid, housing, job placement or education
- Arranges follow-up appointment at patient's local mental health center prior to discharge. Coordinates with mental health centers and other human service resources pertaining to specific problems and needs.
- Makes referrals to appropriate agencies for support services before date of discharge.
- Meets with patient's Continuity of Care Representative when scheduled.
- Coordinates completion of required forms within prescribed timeframe of discharge according to established guidelines.
- Develops and maintains a network of community contacts.
- Prepares reports for supervisors, colleagues, or outside requests and disseminates to
- Prepares quality improvement report for Social Work Department or other authority on a regularly scheduled basis.
- Prepares cross audit for social services records on a regular basis.
- Prepares continuity of care report on a regularly scheduled basis.
- Prepares other reports as requested.
- Maintains knowledge of current trends and developments in the field by reading appropriate books, journals and other literature and attending related seminars, conferences and the like. Uses knowledge to assist in in-service training development.
- Attends seminars and professional meetings when available.
- Maintains a continuing education program by attending in-service training when scheduled.
- Maintains professional knowledge by reading up-to-date articles, books, etc.
- Completes all coursework or clinical training necessary to keep licensure current.
- Develops and provides appropriate in-service training, as needed.
